Insurance can be very confusing. The many kinds of health insurance have their own requirements. Our staff will be happy to review your individual coverage on the day of your visit or over the phone. Please call us at (319)398-6172 or visit our office for answers to your insurance questions.

We ask that each time you come to the office you bring your insurance card with you. We need to keep your records current to prevent problems with insurance payments. If we do not have your insurance information, you will be billed for our services.

Many insurances require a copayment for each office visit. If your insurance company requires this, the fee is due at the time of your visit.

If you do not have health insurance coverage, please plan to pay at the time of service. If you are not able to pay your bill in full, our billing office will work with you to set up a payment plan.

We accept cash, checks and credit cards for your convenience.

Please tell the receptionist when you check in that you are being seen for a condition resulting from a Worker’s Compensation claim or a motor vehicle accident.
