How do I request appointments online?

If you have already created a User ID and password, you can log in and request an appointment by clicking on Appointment Request. Select your doctor and the Family Practice Center location and complete the required fields.

We will make every attempt to reply to your routine appointment request in a timely manner. Once we have processed your request you will either receive an e-mail asking you to log onto the website to securely receive our response.

If you (or your child) are sick and want to be seen immediately, it is best to call the office between 8AM and 5PM at 398-6170.
This feature is for NON-EMERGENCY, routine requests only. If you have an emergency call the office or 911 immediately!
